Before You Cosign
You are being asked to guarantee this debt. If the primary borrower does not pay the debt, you will have to. Be sure you can afford to pay if necessary and that you want to accept this responsibility. You may have to pay up to the full amount of the debt if the primary borrower does not pay. You may also have to pay late fees or collection costs, which increase this amount.

The lender or other holder of this Note can collect its debt from you without first trying to collect from the borrower. By signing the promissory note as Cosigner, you would waive any right to demand us to commence a collection action against the borrower within three months as provided in the Official Code of Georgia Annotated, Sec. 10-7-24.The lender or other holder of this Note can use the same collection methods against you that can be used against the borrower, including suing you or garnishing your wages. If this debt is ever delinquent or in default, that fact may become part of your credit record. This notice is not the contractual provision that makes you liable for the debt.

Verification of Customer Identity
Federal laws and regulations require us to request information from you prior to opening an account or adding an additional signatory to an account. The information we request may vary depending on the circumstances, but at a minimum, will include your name, address, an identification number such as your social security or taxpayer identification number, and for individuals, your date of birth. We are also required to verify the information you provide to us. This verification process may require you to provide us with supporting documentation that we deem appropriate. We may also seek to verify the information by other means. We reserve the right to request additional information and/or signatures from you from time to time.

Credit Authorization
I authorize Citizens Bank and any successors or assigns to obtain a consumer report and non-credit data to make a loan decision, to confirm my identity and/or to reduce the risk of fraud. Citizens Bank will attempt to verify the information that I provided in my loan application, including any employment and income history, and I expressly authorize all such entities to provide you any information that you deem relevant to my application for this loan. If I request it, I will be provided the name and address of the consumer reporting agency that furnished the report.

I agree that everything I provided on this application is accurate and complete to the best of my knowledge and that I will have to accept the terms and conditions in my Promissory Note before my loan can be funded.

You, your agents and the student's school(s) may receive, provide or confirm information about this application, loan account, the student's attendance, financial aid or status to the cosigner (if any) and to others in accordance with any applicable laws.

I authorize you (and your affiliates, agents and contractors) to contact me at any number I provide to you, from which I call you, or at which you believe you can reach me, and that you may contact me for any reason, including regarding any request I make for a loan or other product, to service or collect on every account I currently have with you or may have with you in the future and regarding any other products or services you are providing to me or which you provide to me in the future. I authorize you to contact me in any manner, including by means of automated dialing devices, prerecorded messages, wireless push notifications, or text messages, even if I am charged for receiving the communication and even if I will receive the communication on a mobile or wireless device. I agree that such calls are not unsolicited and that you may monitor and/or record them.

State Disclosure Notice

CALIFORNIA RESIDENTS ONLY: A married applicant may apply for a separate account.

MASSACHUSETTS RESIDENTS ONLY: Massachusetts law prohibits discrimination based upon marital status or sexual orientation.

NEW YORK RESIDENTS ONLY: A consumer credit report will be requested upon submission of my application and subsequently for the purpose of reviewing any account originated pursuant to this application, to evaluate my credit worthiness prior to releasing loan funds to my school, for the purpose of taking collection action on the account(s), or for any other legitimate business purposes associated with the account(s). Upon my request, I will be informed whether or not the consumer report was requested and the name and address of the consumer reporting agency that furnished the report(s). Please note that you will request subsequent reports regarding the credit, if such credit is granted and accepted.

No provision of any marital property Note (pre-marital Note), unilateral statement under Section 766.59 or court decree under Section 766.70 adversely affects the interest of the Lender unless the Lender, prior to the time that the loan is approved, is furnished with a copy of the Note, statement, or decree or has actual knowledge of the adverse provision when the obligation to the Lender is incurred.
